National Breakfast Club Survey

Dear Parents/Carers,

The National School Breakfast Programme (NSBP) provides food and support for our breakfast provision. As part of our involvement in the programme, we have been asked to gather your views and feedback about the impact of breakfast on your child(ren).

We would appreciate you taking the time to complete this short questionnaire so that the team can gather your feedback on the breakfast provision at our school.  It should take no longer than 5 minutes. 

The questionnaire is anonymous and is being conducted by the NSBP. It is independent of our school. The NSBP may use your feedback in marketing and publicity, but it will always be anonymous unless they have obtained your consent beforehand.

The feedback will help the NSBP to measure the impact that the programme is having and help them to develop school breakfast provision in the future. The overall feedback results will also be shared with us, so that we too can use it to better understand and develop the breakfast offer to our pupils.

For the purposes of the questionnaire, if you have more than one child at our school, please answer it for the oldest child.
